Cases are assigned to counties by zip code. When local public health departments complete the case interview, the county of residence is verified and the county case number is adjusted accordingly.

Coronavirus Disease 2019 (COVID-19) Information


Horn Memorial Hospital is closely following information provided by the Centers for Disease Control and Prevention (CDC), and the Iowa Department of Public Health (IDPH), to help keep our patients and families informed about the Coronavirus Disease 2019 (COVID-19). COVID-19 is a respiratory illness that can spread from person to person. HMH has comprehensive infection control policies and procedures in place, and remains committed to keeping our patients, staff and community safe.

Updated Horn Memorial Hospital Visitor Restrictions:


Beginning Thursday, March 19, 2020, Horn Memorial Hospital (HMH) is implementing a screening protocol for all patients, staff and visitors that enter the hospital. The front entrance and the Emergency Room door will be the only access to Horn Memorial Hospital. Please use the Emergency entrance for EMERGENCIES ONLY.

The front entrance will be open 7:00 a.m. - 7:00 p.m. Monday through Friday and 8:30 a.m. - 5:00 p.m. Saturday and Sunday. Staff will be present at this entrance to screen all that enter

The hospital continues to have visitor restrictions in place dependent on census, staffing and isolation status.


New policies are now in place to keep our environment safe for patients, staff and visitors, they include:


The hospital cafeteria is closed to the public until further notice.

Persons with flu-like symptoms will not be allowed to visit patients or patient care areas.

It is critically important to follow recommendations made by the Centers for Disease Control and Prevention (CDC) and other public health officials to prevent the spread of respiratory diseases like COVID-19, such as:

Avoid close contact with people who are sick.

Stay home when you are sick.

Avoid touching your eyes, nose, and mouth with unwashed hands.

Wash your hands often with soap and water for at least 20 seconds. Use an alcohol-based hand sanitizer that contains at least 60% alcohol if soap and water are not available.

Sneeze and Cough into your sleeve or a tissue.

Clean and disinfect frequently touched objects and surfaces.

Updated Protocol for Horn Physicians Clinics:


Please call us prior to presenting to the office. You will be screened over the phone, and we will instruct you on what to do.

All patients presenting to Horn Physicians Clinic are asked to call when they arrive and to stay in their car.


In order to protect our staff and patients, all persons that enter HPC will be screened. If you have a fever or respiratory symptoms we are requiring you to put on a mask.

All exam rooms are being disinfected between every patient.

COVID-19 Testing Prices

If you do not meet the Iowa Department of Public Health criteria for COVID-19 testing at the State Hygienic Lab, the cash price for a COVID-19 test administered upon the direction of your practitioner is $204.00. The price for a Serology IgG (antibody) test is $104.00.


The CDC recommends that you seek medical advice if you are ill with fever, cough, or have difficulty breathing.

Monkeypox Information


Monkeypox was identified in the United States on May 18, 2022. According to the Centers for Disease Control, the United States has over 6,000 reported cases. As of August 2 there are 10 reported cases in Iowa.

Monkeypox symptoms are similar to smallpox. Most people with monkeypox develop a rash that may look like pimples or blisters, become pus filled, scab and fall off. The rash may be painful or itchy and can be located on hands, feet, chest, face or mouth, or near the genitals. In addition, some people will also have a fever, headache, muscle aches, backache, swollen lymph nodes, chills, exhaustion, sore throat, nasal congestion or cough. Symptoms start within three weeks of exposure to the virus and last for two to four weeks.

Transmission occurs through close physical contact (direct contact with rash, scabs, body fluids and respiratory secretions with prolonged face-to-face contact), or by using or touching objects, fabrics (bedding, towels, clothing) and surfaces not disinfected after use by someone with monkeypox. Monkeypox is not a sexually transmitted disease, but close physical contact during sex is one way the virus can spread. Monkeypox is contagious from the start of symptoms until scabs fall off and new skin forms.

One of the ways to protect yourself and prevent the spread of monkeypox is to avoid skin-to-skin contact with people who have been diagnosed with or have a rash that looks like monkeypox. Do not touch the rash or scabs. Do not kiss, hug, cuddle or have sex with someone who has monkeypox. Avoid contact with objects and materials that a person with monkeypox has used. Do not share eating utensils or cups. Do not handle or touch bedding, towels or clothing of a person with monkeypox. Objects and materials touched by a person with monkeypox should be washed with soap and water and disinfected before used by other people. Wash your hands often, including before eating, touching your face, and after using the bathroom.
